Co-Management
Co-Management with Dr. Stewart Shofner
Dr. Bagley has Co-managed LASIK surgery with Dr. Shofner since 2003.
Dr. Shofner performs LASIK surgery in Nashville.
Dr. Bagley performs the Pre-Operative exam at Perfect Optical Eyecare Center in Huntsville.
After surgery, Dr. Bagley performs the post-operative exams also at Perfect Optical in Huntsville.
Co-Management Fee's
Co-management fee's are subtracted from the total fee's quoted for Lasik Surgery.
Current Fee Structure with Dr. Shofner and Perfect Optical Eyecare Center
Custom Lasik Surgery $800.00
Traditional Lasik Surgery $600.00
Extended Plans require annual comprehensive eye exams to keep the plan active. Annual Exams are not covered. Usual and Customary exam fees apply.
If you require an enhancement, per and post operative care are covered within 1 year after your original Lasik Surgery. Beyond 1 year after your original surgery, we charge $300.00 for the pre and post operative care.
Scheduling Appointments
All appointments in Nashville should be scheduled directly with Dr. Shofners office in Nashville. 1-615-340-4733

We work together in Lasik Co-management, but we are separate practices and do not have access to each others schedules.

Dr. Bagley has Co-managed LASIK surgery since 2003.
Dr. David Judge performs LASIK surgery in Huntsville with Vision America.
Dr. Bagley performs the Pre-Operative exam at Perfect Optical Eyecare Center in Huntsville.
After surgery, Dr. Bagley performs the post-operative exams also at Perfect Optical in Huntsville.
Co-Management Fee's
$720.00 Subtracted from the total fee. This includes the Pre-operative and Post-Operative Care.
For Example: $3598.00 total fee. $2878.00 Paid to Vision America. $720.00 Paid to Perfect Optical.
With VSP discount Co-management fee's are $640.00. Includes Pre-operative and Post Operative Care.
For Example: $3190.00 total fee. $2550.00 Paid to Vision America. $640.00 Paid to Perfect Optical.
Post-Operative Care is typically for 3 months. These fee's will cover up to 1 year of Post-Operative Care.
